What Did You Know and When?
If a resident falls, how quickly do you know about it? Are staff members aware of residents who show a change of condition? Do they know who is on duty and what their responsibilities are?
One of the biggest issues in assisted living and long term care facilities is communication. That’s because there is a lot going on in every community, and a lot of change in residents’ lives. Without clear communication and accountability, staff risk compromising the quality of care that residents deserve.
Introducing the Community Quality Board (CQB)
The CQB is a foundational practice of the LiveWell Method. It is a board located in a visible place that shows important information that care staff, residents, management, and families need every day, every shift.
The CQB is a visual management practice that uses visual tools to give care staff the knowledge they need within three seconds - what we call The Three Second Rule. The CQB answers these questions and more:
How many falls happened this month? Is that number increasing or decreasing?
Are staff getting their lunch breaks?
Who is visiting today?
Do we have any safety drills scheduled?
Do any of our residents need extra care today?
Do any of our team members need extra care today?
Do we have any unexpected staff absences today?
Communities that have CQBs are able to:
Ensure that everyone understands what’s happening in the community
Encourage open discussions about successes and areas for improvement.
Foster a culture of teamwork and support.
Improve quality of care.
Build trust among staff, residents, and families.
How to Get Started:
Download the sample CQB linked below.
Create your board using butcher paper or cardboard
Choose a visible spot for your board where everyone can see it.
Fill it out with important measures like falls, medication errors, and staff training.
Include compliments given by care staff to other care staff.
